I understand that the session should not be construed as a substitute for medical examination, diagnosis or medical treatment. I understand that Skin Perfect LLC is not qualified to perform, diagnose, prescribe or treat any physical or mental illness and that nothing said in the course of the session given should be construed as such.

Because certain treatments should not be performed under certain medical conditions, I affirm that I have stated all of my known medical conditions and answered all questions honestly. I agree to keep Skin Perfect LLC updated as to any changes in my medical profile and understand that there shall be no liability on their part should I fail to do so.

I also understand that Skin Perfect LLC reserves the right to refuse to perform treatments on anyone whom they deem to have a condition for which treatments are contraindicated. 

Since multiple treatments may be required, this medical form and all consents signed continue for all subsequent treatments by Skin Perfect LLC, regardless of the time between treatments.

TATTOO REMOVAL Consults | Treatments • 

We can’t give a TOTAL # of treatments, as there are too many variations that determine the lifting process. Some variants are:

Color of Tattoo - What actual color the Artist used, was it a mix of 2/3/4 colors.

Did Artist use Semi +/or Permanent Coloring or a a Mix

Scarring on the Skin - Determines where we can use laser settings not to pop skin. The Artists can go to deep or dragging color that causes trauma to skin and will be a silverfish coloring when the pigment starts lifting.

During the Lifting process we like for our Clients to stay as healthy as possible, since the immune system absorbs the break in color and pushes it through our waste system. Highly recommended to take approx 1000 mg of Vit C few days before we start the first TRX. -If health reasons prevent you from this or questions around this please seek advice from your primary care physician.
